The age of the child should be as mentioned below on April 1st, 2024.

Important Guidelines For Filling Up The Form
- 1 . All fields marked with asterisk (*) are mandatory. In case any field is not applicable, mention NA.
- 2 . The incomplete registration form and those with incorrect information will automatically stand rejected.
- 3 . Fill all the particulars of the form accurately and carefully.
- 4 . The date of birth filled in the form must tally with the one recorded in the Date of Birth certificate issued by the Municipal Office.
- 5 . You will receive confirmation email on submission of the form.
- 6 . Keep the Xeroxed/Photostat copies of the listed documents ready at the time of admission.
- a. 6 Passport size latest photographs of the child and 2 Passport size latest photographs of both parents.
- b. Attested copies of legal document if the child is being brought up by a single parent.
- c. Birth Certificate/ Passport of the child.
- d. Proof of residence (Aadhaar card/passport/ voter id/ utility bill/ rent agreement/sales deed/ ration card).
- e. Photo id proof of both the parents (Aadhaar card/passport/ voter id / driving license).
- f. Academic transcript of last school attended
- g. Vaccination card/Medical certificate of the child.
- h. Original transfer certificate from last school attended
Note: – Documents to be brought in original and self-attested photocopies at the time of admission.
